The sever I play on is crashing it think is coursed by a rapid loading of chunk , elytra and nether portals. It’s causing roll backs which can delete items and duplicate items also crashing around a hopper mine carts just deletes the hopper mine cart . They may be moving at the time of the crash so that might change how it works. I think it can be done consistently as it happened multiple times under the same circumstances
