This is a strange bug. I initially thought it was an isolated incident, but then I discovered how to replicate it consistently. This bug has been around for many versions, I've lost count. I haven't tried to exploit this bug on public servers because I don't want to experience this error and "lose" my data, so I do it (not intentionally, I just discovered it) on dedicated servers.
The bug is that when you leave the connected server, you suspend the app (you exit the app, leave it in the background, and then return). When you reconnect to the server, you'll enter a completely new player profile, even though the gamertag, XUID, and PFID will be exactly the same. Your inventory is reset. The thing is, this isn't just a simple /clear; it's much worse. When you reconnect, you appear in your previous profile, so you have two profiles with different inventories completely working in parallel depending on how you entered the game, whether you suspended it or not (it's completely crazy).

Answering the questions, I would have to create a new server to replicate this bug. The affected devices were my phone and my laptop (host). There were no background processes running on my PC, but when I suspend the game on my phone, it sometimes disconnects from Microsoft services, which prevents me from accessing servers (sometimes, not always). It's like the game goes into offline mode. My server is properly configured; I didn't alter any properties, I only changed the world name and the default difficulty, While I have mobile data enabled on my phone, that's the only difference I have active. (Please note that I do not use mobile data at any of the times when the problem occurs, it's a insolated option enabled)
