When a player's skull is placed onto the last slot of hot bar, the inventory displays black on player's offhand slot, and the status effect tag displays incorrectly(with the black on the side, this is in creative only) as well. Affects both creative and survival players, but differently (see attached images). No effect if another item placed in the last hot bar slot.
