Tools can only break one block

After loading in a survival "buffet" world, any and all tools will break one (1) block before glitching out, causing the block to be unbreakable with same tool.

Steps to reproduce:
1. Spawn, or craft a tool
2. Break two (2) blocks.

Environment

Java Version 8 Update 171 (build 1.8.0_171-b11)
Windows 10
