It doesnt lag but it destroys

When I play Minecraft sometimes my Minecraft is glitching like CRAZY! I don't know how to explain but i have an example: When im holdin like a iron picaxe it turns into a rail but im still holding a picaxe but in my inventory it look like a rail. Its work on all gamemodes..
