Note: I do not know how to recreate the event. All that I know is that sometimes when I use redstone in small areas, it glitches my piston and extends it permanently.
I also make sure no redstone can hit it by surrounding it with glass. I am in singleplayer

but i have no bud switches involved and no 2 currents near that piston. What is the solution? This is also not diagonal. It is two blocks away and passes through glass.

Solution: make sure no active redstone is up to 2 blocks above the piston.
