When swimming in minecraft, reaching the surface causes my minecraft to freeze and requires manually forcing a crash. Log is attached (not sure how helpful that is)
As the game doesn't automatically crash but simply freezes, I can't attach a crash log.

Yes it happens in singleplayer as well, I don't think a crash is going to help much as by the time the glitch occurs I can't interact with minecraft at all. Ignore the large numbers of entities, they're not the cause of the glitch.

The attached log looks like you were on a non-vanilla server. Does this happen in singleplayer or on vanilla servers as well?
Can you also force a crash (hold
F3+Cfor about 10 seconds) and attach the report here please?
