After any death on a hardcore mode server, you can respawn in spectator mode, but once respawned, nothing loads except for mobs and players. You can type in chat for a bit, but eventually the game freezes up, and nothing can be done. It has been tested to have the same result multiple times. It only occurs on hardcore mode servers as I have tested. Nothing wrong on the server side with permissions.
