For some reason in creative and survival the number 1 slot keeps changing and giving you a different block or item. I am not able to use my sword in that spot to battle monsters. I have to place it in another spot. In creative it will switch to Grass or a block that I did not opt in.
I tried using it with and without any mods and even went to default and it is repeating itself. This effects all modes in the game and this is within minecraft itself.

Are you perhaps accidentally clicking the scroll wheel while changing slots, or have you reassigned the pick block key? Or are you pushing the number keys on your keyboard with your inventory open?
I hit the number 1 on the keyboard. It then automatically changes the object to a different block. If I am using a sword in creative It would change it to a grass. Does not matter to what it is. It will change the block totally to something else.
Are you aware of the "Pick Block" function? Have you possibly changed the button for it to the number 1 key?
The Pick Block function of Minecraft is bound by default to the middle mouse button or clicking the scroll wheel. If the block you're pointing at with the cursor is in your hotbar, it will switch which slot is selected to the one with that block. If it's not, and you're in creative mode, it will replace the item in the current slot with whatever you're pointing at. So if you have a sword in your hand, and you're pointing at a grass block, and you push the Pick Block key, it will replace your sword with grass. That's what it sounds like is happening to you.
To reach the controls screen, hit escape, click options, and then controls. "Pick Block" should be in the lower-left corner, and by default is set to "Button 3". What does it say for you? Also, you should probably do as Tails advised: generate a crash report, and attach it here.
