I can't reproduce it 100% of the time but it does happen. It really sucks, happens with both advanced tooltips and normal ones. Attached are my settings.

Yes I can reproduce it without optifine.
There's no steps really then to just have the game running single player for a few hours or trying to close / reopen minecraft.
I don't have a 140+fps camera so I can't really show it.
Hey, this is my issue. I must say, your indexing on google is amazing.
This seems to be related to MCL-12243, I'm not sure if this was ever solved but it seems to be gone on newer versions.
If you did solve it on purpose here then it's possible that the same solution can be applied to that as it was with this.
So I've recently acquired a 144hz monitor, and decided to do a screen capture of the game window running at this refresh rate. Playing it back frame by frame, the tooltips do appear to be animated precisely at the desired FPS.
Can you confirm for me that, by tooltip, you in fact mean the popups such as for advancements?
Also, you say this only occurs some of the time? Does this mean that, in some instances, it does display correctly?
By tooltip I mean what shows when you hover over something in your inventory
