[MC-15439] Swords Randomly Delete themselves while in use

When fighting an enemy after just logging in, sometimes my sword will "disappear", it just goes away. This is very annoying because I am using diamond swords. It has happened twice now.

Also note it was at nearly full durability.

Same thing happened here, it disappeared at the same time my helmet broke, it was brand new.
