This might be unique to the save we're on, on the server, but I've visited 3 outposts for about 2 hours each, and ever since killing the captains at one, they all don't spawn captains anymore.
Normally I'd think it was bad luck but im at outpost #3 and I've stood here for about an hour and a half on normal difficulty.
