Crossbow sometimes won't fire, still uses an arrow

After loading an unenchanted crossbow with normal arrows and attempting to fire it, the arrow disappears and is used up but no arrow actually fires. No sound is played, mobs won't take damage. The crossbow simply seems like it was unloaded. Sometimes the arrow will actually fire, but I'm not doing anything different, it seems like a random chance that it will or will not actually fire.

It happened to me in the Nether before after playing for a bit and it just happened to me again minutes after loading my world.

Steps to replicate
1. Equip crossbow while having arrows in inventory
2. Hold use button to load crossbow
3. Aim crossbow and press use button again

Expected outcome
Arrow is fired in the direction you are aiming

Actual outcome
Crossbow is no longer loaded, arrow is no longer in inventory, but no arrow is fired.
