[MC-172158] shields stop working when going through a portal

If you enter a nether portal or enter an end portal while holding a shield in your offhand you will become unable to block unless you take it out of your inventory and then put it back.

We're actually already tracking this issue in MC-171484, so I resolved and linked this ticket as a duplicate.
