blocks don't place and vanish from the game

When I place a block it won't place and vanishes from the game.

This is fixed by closing and saving the world and reopening-the block will be back in my inventory in it's previous place before I put it in the hot bar to attempt to place it.

Sometimes another block will be placed. For example, If I attempted to place a chest, a cobblestone block will placed instead and the chest will vanish. it can still be 'reset' by closing and saving the world then reopening.

This  can occur with any block seemingly at random, for example torches will not place one time, then next cobblestone until i close and save then reopen the world. It happens often soon after starting the world.

I am playing on survival single player, not on a server.
