Phantoms spawn in mushroom island biomes, a biome which does not spawn hostile mobs.
How to reproduce:
1) Go to a mushroom island
2) Do not sleep for 3 nights
3) Watch as phantoms eat your face

We're actually already tracking this issue in MC-126778, so I resolved and linked this ticket as a duplicate.
