I am unable to recreate this 100% of the time, but sometimes when breaking a grass block with grass on it fails to break the grass. The result is floating grass as in screenshot.

Can you try closing and reopening the world, please? After you've done that, can you check if the grass is still floating, or if there are blocks that reappear there?
