If I hibernate my computer without exiting Minecraft, when I resume the graphics are all bugged out. With the pre-1.6 launcher, the glitchiness is confined to the Minecraft window; with the newer launcher, it also glitches the top of my screen.
Note that my attached video shows what happens with the new launcher. The graphics error at the top of the screen is the only difference between the two that I've seen.

I forced a crash with and without hibernating (they're marked "control" and "experimental", respectively) since I thought you might want to see what my computer was doing when everything was normal.
Poking around a little more- Apparently switching texture packs fixes the issue (until the next hibernate).
Your graphics driver is very old
OpenGL: Intel 945GM GL version 1.4.0 - Build 7.14.10.4926, Intel
Try to get an update (but I'm afraid for XP you won't get any newer version)
Intel hardware / drivers are very error prone regarding OpenGL (and IMO in general...).
But this is a technical support issue; this site is for bug reports only. For technical support please use the Mojang Support Center.
Please force a crash by pressing F3 + C for 10 seconds while ingame and attach the crash report here.
