What is the minecraft:generic.attack_speed attribute value set to for your player? You can check this by running "/attribute @s minecraft:generic.attack_speed get". If this is set to a low number, this can cause your main hand to disappear.

Duplicate of MC-228490, which was later resolved as invalid due to this issue being caused by optifine.
