So I wanted to mine sand in my hardcore world where suddenly I noticed that the part of that beach isn't loaded properly and me and the shield in my offhand has swapped places: I saw myself in front of my screen and me running in my offhand (view videos for reference)

That is quite bizarre. I'm not able to replicate the issue though.
I'm pretty sure the issue with the beach not loading properly is you experiencing MC-250331. I don't know if that shield bug you are experiencing is caused by that same bug or a related issue.
