My world seeded by -1315920500 under release 1.6.2 generates a ladder supported by no block in 488 28 898 inside a mix of a stronghold and a mineshaft.
This causes that ladder has no back texture.

Do you have seed and coordinates of where this used to happen but no longer happens? Because for me, at least on this custom superflat preset, strongholds still generate ladders (and doors) without blocks supporting them:
3;minecraft:bedrock,5*minecraft:stone,5*minecraft:dirt,minecraft:grass;3;stronghold,biome_1,decoration,dungeon,mineshaftseed -5304769350772904252, x: -35, z:-800.

edit2: I did check the provided seed, but in 1.8 no stronghold spawns at that location.
In fact for me this ticket was for the missing back texture issue 😃 There are 2 different issues in this ticket.
You're right, it can be interpreted both ways. Anyway, ladders and doors generating without supporting blocks is WAI, weirdly: MC-40712
