if you get an item stacked to more than 64 through any means such as the one shown in the attachment, then open your inventory and press the swap item with offhand button while hovering over it. The item will be put into the offhand on the client side, but deleted on the server side. It can sometimes be restored on the server side by dragging the item out of the offhand slot

If it can be achieved with commands, it's still valid.
This easily can by setting NBT data of a container.
I attempted to reproduce this problem, but was unable to experience a desync. I used the command provided below to obtain a stack of 127 dirt and follow the steps within this report, but was still unsuccessful.
/summon minecraft:armor_stand ~ ~ ~ {ShowArms:1b,HandItems:[{id:"minecraft:dirt",Count:127b}]}
With this being said, please provide a video demonstrating the problem while the F3 debug screen is enabled.

how does this affect the gameplay, you cannot get 127 stacked items without cheats
