I have attatched a clip of the issue happening. While this clip is shown in the latest snapshot, I had the same issue with a ghast in 1.20.1.
(Please ignore all the redstone block item displays, they are for a custom boss fight I am working on, which is a heavy work in progress)

@Dhranios The setup can be reproduced in vanilla. All you need to do is summon an entity: a ghast, a wither, or probably other mobs that have not been tested, with the NoAI and Rotation tags. So, the command would be: /summon minecraft:ghast ~ ~ ~ {NoAI:1b,Rotation:[90f,0f]} I have attatched a video of those steps being reproduced in a newly created flat world. I was unable to produce the exact same result, however, if you look closely you will see the wither turn its head slightly upon logging in. I am unsure if this is related.

I'm unable to reproduce this. Please can you see if this is still an issue in the latest snapshot and release?
Please provide steps to reproduce in a minimal test setup. Make sure no command blocks/data packs might be modifying the rotation when you leave/re-enter.
