Bottom piston not extending when I build two double piston extenders on top of each other

The bottom piston does not extend, and I am not sure of the issue, it worked back in versions 1.19 and previous, and I am not sure about 1.20. It does not need a block update (unless there is a block update bug, of course), as both the top piston and the redstone dust powering it (redstone dust can give block updates if it is 2 blocks away when it is powered) in theory should give it a block update

I attached a zip file of the world: the affected piston mechanism can be viewed at coordinates -232 6 112, and can be tested by placing and activating a lever at -233 6 113. Placing a block (or giving any block update) right underneath the piston, in block -232 5 109, while the top piston is activated and the bottom isn't but should be will activate the piston. I implemented a temporary fix on the other side of the redstone contraption, at -240 4 111, that gives a block update to the piston 5 redstone ticks (10 game ticks) after it should have extended.

Would MC-11193 describe your issue by any chance? Interestingly, when I break the powered wire prior to the piston extender the contraption works as expected. However, standing on the pressure plate will in fact not make the piston extender work.

It is possibly that bug, but it does not make sense that it would be considering that this bug did not happen before 1.20.

I have however managed to track down your issue and have fixed the door. The problem is that you had a circuit on the left side of the door to update the pistons via redstone block updates from below, but this was missing on the right side:
