I was placing stiicky pistons and quasi move the piston and it made 2 hads then i broke the pisotn and i was left there

You can get this with commands. And sometimes world corruption. Did you download the world?

Using this command you can get that: /fill ~ ~-1 ~ ~ ~-1 ~ minecraft:piston_head
