Place a sticky piston, activated with a redstone block. When not activated upwards or downwards, the issue shown in the figure occurs. Enabling minecart improvements increases the probability, but it can also be reproduced without enabling them, albeit less frequently. The faulty piston head cannot be destroyed; re-entering the save file can restore it.
