[MC-299981] Tools disapearing randomly

Tools disapear randomly out of your inventory.

it happens since i updated the server we play with 5-6 people from 1.21.4 to 1.21.8.

it seems the tools just lose all of their durabilty suddenly when in use. doesnt matter if its an axe without unbreaking or for example unbreaking 5.
