When I was ridding a Happy Ghast with 2 other player I needed to stop the server, but when I and the 2 other player re-joined after I started the server again we spawned in the sky, not seeing the Happy Ghast and fell into a river luckily. However, when we looked if the Happy Ghast was there, it was nowhere to be seen. At first we thought it was a visual glitch, but sadly it wasn’t.
I do use fabric mods on the server, but they all are either performance mods or mods that don’t change mechanics of the game at all. I also highly doubt that the mods are causing the issue.

Yes, this issue does occur even without mods. The environment is a fabric server that has some lightweight optimization mods that do not affect the issue at all. To recreate the issue, you need a java multiplayer server and at least 2 players. Once all the players have connected, you need a full-grown happy ghast that has a harness and both 2 players have to ride it. Once they are on the happy ghast they need to both disconnect while on the happy ghast at roughly the same time. (The time does not really matter.)

However, this issue is Invalid.
Your game, launcher or server is modified.
If you can reproduce the issue in a vanilla environment, please recreate the issue.
Any non-standard client/server/launcher build needs to be taken up with the appropriate team, not Mojang.
A world which had previously been opened with a modified client might be corrupted. Please try reproducing the issue in a newly generated world.
Any plugin issues need to be addressed to the creator of the plugin or resource pack.
If you have problems on large servers like Hypixel, please contact them first as they run modified server software.
