dropped ladders appear as 2d images
i dropped a ladder in creative mode and the item showed a 2d image, even from above.
all non-blockish items act the same (redstone, flint, repeater)

This is intended in "Fast Graphics"
Switch to "Fancy Graphics" if you don't like it.
