That happens with nether warts that have a damage value greater than 2.

Whoopsie, had data values 0-7 registered for Nether Wart when it only uses data values 0-3, so it was still trying to use minecraft:nether_wart#growth_stage3 and such...

I have the same problem... all nether wart that is fullly grown.. appears with the missing texture block.
