Blocks keep reappearing

When I am hammering down on a block from any side of it (doesnt matter what block it is because it has been happening with all my blocks) sometimes it would disappear normally but some times it would instantly reappear like I didnt destroy the block at all. This hasnt happened to me until now and Ive been glitch free since the game came out on Xbox 360
