[MC-52632] losing all achievements

everytime i log into my multiplayer server and open my inventory i get the taking inventory achievement plus sometimes im teleport to where i was a few minutes prior to logging out and occasionally losing items in my inventory

Please force a crash by pressing F3 + C for 10 seconds while in-game and attach the crash report ([minecraft/crash-reports/crash-<DATE>-client.txt|http://hopper.minecraft.net/help/finding-minecraft-data-folder]) here.

Crash log looks sane, but we need the logfile of the server, since this is not a client isue.

Attach the logfile of the server.
You'll find it in <SERVER-DIRECTORY>/logs/<DATE>.gz

can anyone tell me why i have this problem i cant join anyserver except of my own server i have java edition please respond does anyone know why
