Portals don't Work properly

In attempting to use both a Nether and End portal in survival, the player gets teleported to a random location in the proper dimension.

Eg. Nether: I enter the nether portal and suffocate in the ground. Switching into creative, there is no exit portal.
End: On first entering the portal, my player got spawned in void and died instantaneously.
