When dying on a ghost slime block with a cactus underneath, the "UUID not found" error message appears

After creating ghost blocks using pistons and dying from a cactus underneath, the error message "UUID not found" is displayed

Please provide steps to reproduce.
The ability to create "Ghost block" seems to be the underlying bug.
