Lever placed in piston is destroyed when it state changes back to "off"

Steps to reproduce:

• Place a lever in a piston (can only be placed above)

• Activate lever

• Deactivate lever, it breaks.

Sometimes there is also a really fast white flash in the piston update animation when the lever is deactivated. Works on both types of pistons and in all directions. Screenshot attached.

Also, levers can only be placed above pistons and not on the other 5 sides, is this the intended behaviour?
