What is happening:
There is a lever attached to some redstone wire attached to a piston with an unpowered Powered Rail one block below the piston. The unpowered Powered Rail must be at an angle. The redstone wire is also attached to the powered rail. When I turn on the lever, the powered rail and the Piston turn on, but when I turn the lever off, the powered rail turns off and the Piston still has its arm out.
What I expect would happen:
When I turned the lever off, I expected both the powered rail to turn off and the Piston arm to retract.
How to re-create:
P = Piston
B = Polished Andesite Block
W = Block with Redstone Wire on Top
A = Air
R = Powered Rail (must be at an angle)
L = Lever

I do not think this is a duplicate of MC-108.
MC-108 specifically requires an active redstone power source (albeit, not directly connected to the piston).
As you can see in this bug, there are NO active redstone power sources anywhere - the lever is OFF, as you can see that the redstone attached to the lever is unpowered.

There was an active redstone power source, diagonally above the piston. When it turned off it didn't update the piston.
