If you cast an Ender Pearl into the End Gateway portal (the one created after defeating the Ender Dragon, which sends you to the "Outer End"), the Attack Indicator and visible recharge animations do not use the proper Attack Speed.
Any item, weapon, or tool used to attack displays the same speed for both animations (approximately 0.25 sec, or 4 Attack Speed). However, the damage dealt does not seem to be affected by the indicator; spam clicking still deals less damage than waiting for the (actual, not the visual) recharge duration.
Teleporting back through the Gateway and switching game modes don't fix the problem, however dying or teleporting to the Overworld do.

I think it's slightly more complicated, e.g. I gave myself an item with full attack speed:
/give @a minecraft:iron_sword 1 0 {display:{Name:"Spammable unbreakable sword"},Unbreakable:1,AttributeModifiers:[{AttributeName:"generic.attackSpeed",Name:"Speed",Slot:"mainhand",Amount:1020.0,Operation:0,UUIDLeast:111l,UUIDMost:111l},{AttributeName:"generic.attackDamage",Name:"Damage",Slot:"mainhand",Amount:4.0,Operation:0,UUIDLeast:222l,UUIDMost:222l}]}which has zero cooldown, and sometimes after passing through the end gateway, my attack indicator goes away for a wooden sword (as though the wooden sword has no cooldown, though it is still dealing correct amounts of damage). I wonder if something like the last tool held/used becomes the 'permanent' attack indicator speed until you reset your client.

It could be that way Brian, It could also only affect items of the with the same generic attack speed (E.G. two swords.) or it could also adversely affect the off-hand item so that It matches the attack speed or damage of another item in the inventory. Regardless, this easily allows for exploits in pvp.
