Key doesn't respawn in the Frost Fortress after using it and then dying

In the Frost Fortress I used the yellow key for the first gate and then died before getting the second one. Now the gate is closed again and the key did not respawn and I can no longer get through the gates.

And the spining wall baldes also went missing after dying, but the sounds still play.
