Ocassionaly after breaking a block you recently placed, the item form appears about 4 blocks away from original location, making it hard to locate after breaking it.

Cleaning up old tickets: This ticket had been set to 'Awaiting Response', but has not received a response from the reporter (~3 months+) so is being closed as Incomplete.
