I lost everything, armor, weapons, and inventory when I died. It never dropped and my wife was standing right next to me and didnt get any of my drops. 10's of hours of work gone. awesome to deal with after a bad day of work, thanks.

I recommend what Jonathan said above. Are you 100% sure it all did not just land in some hole below you? How did you die?
Have you had this issue later in the latest version? If so, can you add it to the affected versions?
