Player transitioned from Nether to Overworld using a portal that had been used many times before, screen froze on loading screen and played injury sound repeatedly. After approximately 30 seconds player was informed of character death. Player respawned and went back to go collect gear and no gear was present in either Nether or Overworld. The gear was netherite with several enchantments on each piece and represented a significant amount of time investment. Please advise.
