In my Minecraft realm, whenever my friends and I cross the Exit portal from the End, it instakills and dissapears all of the items we were carrying at the moment

This exact thing happened to me. Had been on my first tour in the end and got my elytra and was heading home and poof, no gear and can't get any items back
