Losing All Items When Exiting The End

So me and my friends just beat the Ender Dragon on our world, so then we got the egg and left. For some unknown reason, my friend died while jumping into the portal and couldn't respawn. He left the game and spawned in without his items. Then I went through the portal and didn't see any of his items. They weren't in the overworld either. I went in the portal, but I also died automatically, and couldn't respawn. But there was some weird glitch where even though the death screen showed up, enemies could still attack me. The silverfish from the spawner then attacked me, but even though I died from the end portal, I still had my items for some reason, and the silverfish knocked me in lava and I lost all of my items because of this bug. Please fix this. My camera software was not working so I couldn't provide a screenshot.
