Horses don’t always show their actual full range of hearts. If you take a horse to the nether, those ‘missing’ hearts will reappear. Once you return to normal world those hearts remain but disappear again if you get off the horse or leave it a very short while. Note this doesn’t happen with every horse, just some.
Foals can escape through crimson wood gates. Haven’t tested crimson fences but who knows
Sometimes horses will not re enter a nether gate back to the normal world. Leaving the nether and returning usually fixes it
