Duplicating items in hot bar

Hi. Whenever I put an item in my hot bar, it doesn't stay or it duplicates across the open slots. For example, I put one sword in the first slot, it either disappears into my inventory or duplicates into two or three swords and fills up the hot bar pretty quick. Hard to fight anything correctly if I go for the sword and it's not there. Hope this is the right place for this.
