Game goes back to menu when joining second player in split screen

The game crashes to the menu when joining players in split screen. This problem has been occurring for a week or two. It was ok before. After the crash it then says on the save choice screen that I cannot play a multiplayer game because of parental control (parental control is set correctly and allows multiplayer). When I exit the game, log out other users on the console and re-run the game then the multiplayer error message disappears on the save choice screen but usually it all happens again when I join with another player. This happens quite randomly. Sometime we can join with 3 players and sometimes the error appears immediately when the second player joins. This is very annoying.
