Name tags appear to be non existant in dungeons, nether fortresses, mine shafts, and have not been caught while fishing (over 500 other items caught).

Confirmed in survival. I have spent the last 2 days fishing with a luck of the sea 3 enchanted rod, I have caught every possible item 10 times over but no name tag.
