so what happened was me and my friend were playing on one of our worlds and when he went through the nether portal it sent him into a wall and suffocated him and lost all his gear. we have no clue on how this happened

Did you return to his death location before the despawn timer ended?
If you managed to retuen before 5 mins, then you have found a bug that others found.
Dont use portals, they can send you to the wrong coordinates or spawn you at y=256
