Was flying home in the nether and noticed a piglin on top of the nether hub, which is spawn proofed with lower slabs. Only one online at the time, had just entered those chunks.

I have not seen this occur outside of realms, but due to the unpredictability of the bug I can't confirm it occurs only within realms.
