I experienced this today within snapshot 19w12b (server jar). I killed several Illager Captains at an outpost and no bad omen affect. I am not sure if it is intended behaviour? Possibly the captains were outside of the bounding box of the outpost, and you need to kill them within it...not sure how that would work with the patrols though...
